Family Houston is seeking a compassionate and knowledgeable Bilingual Community Support Specialist to join our team. In this role, you will work directly with individuals and families within the community, providing vital support by connecting them to essential resources and assisting with medical, dental, and food benefits applications. Your primary goal will be to help clients create a pathway toward self-sufficiency by ensuring they have access to the services and benefits they need. Bilingual proficiency in Spanish and English is required. The position also requires the ability to travel and work from different office locations as necessary.
Essential Job Functions
Resource Connection
- Assist individuals and families in identifying and accessing community resources to meet their immediate and long-term needs.
- Provide information and referrals to various social services, including healthcare, food assistance, and financial support.
Application Assistance
- Guide clients through the application process for benefits, including:
- SNAP Food Benefits
- Texas Medicaid
- Texas Health Women
- TANF (Temporary Assistance for Needy Families)
- Harris Health Gold Card
- Ensure that applications are completed accurately and submitted in a timely manner.
- Follow up with clients to ensure they receive the benefits for which they are eligible.
Self-Sufficiency Planning
- Work with clients to develop personalized plans aimed at achieving self-sufficiency.
- Provide ongoing support and coaching to help clients stay on track with their goals.
- Monitor and evaluate client progress, adjusting plans as needed to ensure success.
Client Advocacy
- Advocate on behalf of clients to ensure they receive the resources and benefits they need.
- Collaborate with other community organizations and service providers to coordinate comprehensive care for clients.
Documentation and Reporting
- Maintain accurate records of all client interactions, services provided, and outcomes achieved.
- Track and document outcomes and metrics to measure the effectiveness of services provided and ensure that program goals are met.
Professionalism and Confidentiality
- Adhere to agency policies and procedures, always maintaining a high standard of professionalism.
- Maintain client confidentiality in accordance with agency expectations and requirements.
- Be resourceful and proactive in addressing issues that arise, ensuring a calm and compassionate approach under pressure.
Experience, Education, and Knowledge
- Experience in social services, case management, or a related field is preferred.
- Familiar with the application processes for SNAP, Medicaid, Texas Health Women, TANF, and Harris Health Gold Card.
- Experience working with diverse populations, including low-income individuals and families.
- Have a strong understanding of public assistance programs and eligibility requirements.
- Excellent communication and interpersonal skills.
- Have the ability to build trust and rapport with clients from diverse backgrounds.
- Strong organizational and time-management abilities.
- Proficient in Microsoft Office and client management software.
- Fluent in English and Spanish required.
- A bachelor’s degree in social work, human services, or a related field is preferred.
- Equivalent work experience in a similar role will also be considered.
Work Hours and Location
This role operates on a hybrid schedule, with in-office hours from 8:00 AM to 5:00 PM. This role is based at our central office in Houston. The position may require occasional evening or weekend hours, depending on the organization’s needs. Must be able to travel and work from different office locations as necessary.
Compensation
This is a salary, exempt position with a range of $52,000 – $55,000/annually commensurate with relevant skills and experience. Benefits include full medical, dental, and vision benefits, paid time off, short-term and long-term disability and 403b retirement plan.
